Managing whitlows typically does not require medical treatment. However, herpetic whitlows cannot be cured entirely. Nevertheless, the use of oral antiviral medications and topical ointments can effectively minimize the intensity and duration of outbreaks. Moreover, these medications play a crucial role in reducing the risk of transmitting the virus to others. For individuals experiencing frequent recurrent infections, suppressive antiviral medications are available. It is important to initiate medication within 48 hours of the initial symptoms.


Diagnosis

Diagnosing herpetic whitlow involves a combination of observations, patient history, and lab tests. Initially, doctors will make a diagnosis based on the presence of a rash or blisters on the thumb or fingers. To confirm the diagnosis, the physician will collect a swab of the fluid from the blisters and send it for a viral culture test. Alternatively, a blood test can also be used to confirm the presence of the herpes simplex virus (HSV), although the vesicle fluid provides the highest concentration of the virus.
